Eligibility

A patient may qualify for hospice when a physician determines that the patient has a life expectancy of six months or less if the illness follows its expected course. Hospice may be appropriate when the focus of care shifts from curing the terminal illness to comfort, symptom management and quality of life.

Admission Process

A patient, family member, physician or facility may contact Bonum.

Bonum reviews the referral and available medical information.

A hospice clinician conducts an evaluation.

The hospice physician and attending physician, when applicable, review eligibility.

The patient or authorised representative elects hospice care.

The interdisciplinary team develops the plan of care.

Four Levels of Hospice Care

Hospice care is provided at different levels according to each patient’s circumstances and clinical needs. Higher levels of hospice care are provided only when applicable eligibility criteria are met and the patient’s condition requires that level of care.

Routine Home Care

Routine home care is provided when a patient’s hospice needs can be managed in their usual residence. Services are delivered according to the patient’s hospice plan of care and may include nursing, aide services, medications, supplies, and other appropriate hospice services.

Continuous Home Care

Continuous home care may be provided for a limited period when a patient experiences acute symptoms that require more intensive nursing care in the home. This level is provided only when the patient meets applicable eligibility criteria and the clinical circumstances warrant continuous care.

Inpatient Respite Care

Inpatient respite care provides temporary relief for the patient’s primary caregiver through short-term inpatient care. This level is provided when applicable requirements are met and the patient qualifies for respite care under the hospice benefit.

Care Based on Clinical Needs

The appropriate level of hospice care depends on the patient’s individual circumstances, condition, and identified care needs. Bonum Hospice Services coordinates the level of care according to applicable requirements and the patient’s hospice plan of care rather than assuming that a higher level is appropriate for every patient.

24/7 Availability

Patients and caregivers can contact the hospice clinical team 24 hours a day for symptom concerns, changes in condition and urgent hospice-related guidance. Visits are scheduled according to the patient’s assessed needs and plan of care.
